Accepted Students Info
Accepted students into the OT Program must return a signed commitment letter, signed Technical Standards/Essential Functions letter, and must pay a $500 non-refundable deposit in order to hold their seat for the next incoming class.
Each student will be provided additional information regarding these items. Each student will need to submit these items prior to the start of the semester:
- TB screening and Immunization records
- Proof of Healthcare BLS certification
- Completion of Criminal Background check
- Completion of Name Tag form
- Completion of OT shirt order form
- Optional participation in Community Adopt a Family program form
- Vehicle registration form
Each student will be provided the following information by the program prior to start of the semester:
- Housing information
- Orientation schedule
- Fall booklist
- Computer Specifications
- Advisor list
- OT Group Assignments
- Locker Assignments
- SHS Policy for Attire and Professional Presentation
